Common Paper's one-page amendment for changing a signed agreement: the agreement and parties, when the change takes effect, one row per change, and signatures.
A contract amendment is a short signed document that changes specific terms of an agreement the parties have already signed, while the rest of the agreement stays in force. This is Common Paper's Amendment, shown word for word as Common Paper publishes it, without a version number.
Use the Contract Amendment to change a price, extend a term, adjust scope or update a defined term in any signed agreement, not only Common Paper's. To add new purchases under a Cloud Service Agreement, a new SaaS Order Form is usually simpler. Check the original agreement's rules for changes first, because many agreements accept only a written amendment signed by both sides.
How to fill it in
Identify the agreement. Enter the name of the agreement you are changing, both parties' names exactly as they appear in it, and its original effective date.
Set the amendment date. Enter the date the changes take effect. It can differ from the date you sign.
Describe each change. Add one row per change. Name the section or defined term in the left column, and in the right column write the new wording or what is added or removed.
Sign for both companies. Have a person authorized under the original agreement sign for each company, with their printed name, title, notice address and date.

Frequently asked questions
Does a contract amendment need to be signed by both parties?
Usually yes. Most business contracts say they can be changed only by a written document signed by both sides, and the Common Paper Amendment has a signature block for each party. Check the original agreement's amendment clause, since it controls how changes must be made.
What is the difference between an amendment and an addendum?
An amendment changes terms that are already in a signed agreement. An addendum adds new terms that the agreement did not cover, such as Common Paper's AI Addendum for AI features. People often use the two words loosely, so check what the document does.
How do you write the changes in a contract amendment?
Name exactly what changes and what replaces it. Point to the section number or defined term, then give the full new wording, or say what is added or deleted. The Common Paper Amendment uses one row per change, so each change stands on its own and is easy to find later.
